Outlook 2013 keeps prompting for password

My client has Office 365. The users that are on Office 2013 frequently get a password prompt while in Outlook. The dialog box shows:
"Sign in Call us overprotective, but we need to verify your account again before opening this document"

I tried all the tricks listed in these articles with little help:

I had to correct this on a case by case basis. Users that connect SharePoint lists to Outlook were the primary culprits. I teach users this in my SharePoint training so it is widespread throughout the organization.

Right-clicking on the SharePoint lists and selecting "Close" solved the issues for us. As soon as users select Connect to Outlook, the sign-in problem and Send / Receive errors continue.

Our network setup, proxy, circuit, routes all changed which seemed to start these issues. Microsoft also changes their IP addresses to their cloud services. Our network team often has to update route and firewalls when this happens.

You may see this warning when removing lists attached to Outlook 2013:

A contributing factor seems to be the Lync 2013 client is also having some trouble connecting for some clients. Many users are receiving "There are Exchange connectivity issues. Your conversation history cannot be retrieved."
